Originally from Cuba and raised in Florida, Dr. Hernandez is a board-certified Internal Medicine physician with specialized fellowship training in Geriatric Medicine from Yale School of Medicine. She is dedicated to helping older adults maintain their independence, improve their quality of life, and age as healthfully as possible through compassionate, personalized primary care. She believes the best healthcare begins by listening to each patient’s story and partnering with them to create care plans centered on what matters most to them.
After completing her undergraduate education at the University of Miami and earning her medical degree from Florida International University, followed by her Internal Medicine residency and Geriatric Medicine fellowship at Yale School of Medicine, Dr. Hernandez returned to her home state of Florida to care for the community that helped shape her. She provides comprehensive primary care for adults and older adults, with expertise in healthy aging, memory disorders, medication management, falls prevention, and the care of patients with multiple chronic conditions. Fluent in both English and Spanish, she is committed to building lasting relationships with patients and their families while emphasizing prevention, education, and coordinated, evidence-based care.
Outside of medicine, Dr. Hernandez enjoys spending time with her family, fishing, relaxing at the beach, trying new restaurants, traveling, and growing her personal library. An avid reader and lifelong learner, she brings the same curiosity, compassion, and dedication to understanding her patients that she brings to every aspect of her life.
